If you or someone you care for is an older Australian living at home, there’s a government
funding program you need to know about and most people still don’t.
From 1 November 2025, the Australian Government launched the Assistive Technology and
Home Modifications (AT-HM) Scheme as part of the new Support at Home program. It offers
up to $15,000 in dedicated funding for mobility equipment, bathroom aids, home
modifications, and more and it sits completely separate from your other aged care services
budget.
This isn’t a voucher. It doesn’t come out of your quarterly service budget. It’s a standalone pool
of money specifically for the equipment and home changes that help you stay safe and
independent at home.
What Is the AT-HM Scheme?
The AT-HM Scheme is a lifetime funding program that provides older Australians with direct access to
two types of support:
– Assistive Technology (AT): Equipment and devices that make daily tasks easier like walking
frames, shower chairs, mobility scooters, or fall detection pendants.
– Home Modifications (HM): Physical changes to your home that improve safety and accessibility like ramps, grab rails, wider doorways, and more.
Before this scheme existed, many people had to save up from their quarterly Home Care Package
budget to afford equipment. A slow and frustrating process. The AT-HM Scheme changes that by
making funding upfront and dedicated.
How Much Can You Access?
Funding is available for a 12-month period, with the option to extend to 24 months for higher-tier
needs. In exceptional cases, additional funding beyond $15,000 can be approved through a Support
Plan Review.
Important: If you transitioned from a Home Care Package, any unspent HCP funds will be used first
before AT-HM scheme funding is drawn on.
Who Is Eligible?
The AT-HM Scheme is available to older Australians approved for the Support at Home program
through My Aged Care. Eligibility is determined at your aged care assessment — noted in your Notice of
Decision and support plan.
Not currently eligible: CHSP clients cannot access the AT-HM Scheme unless they transition to
Support at Home. Some CHSP participants may access personal alarms via GEAT2GO.
What Equipment Is Covered?
The AT-HM List covers hundreds of products across three tiers:
Low Risk — Simple items, no prescription needed.
- Walking sticks and canes
- Bed rails and positioning aids
- Grab rails and non-slip mats
- Basic shower chairs and commodes
Under Advice — Benefits from health professional input for fit and safe use.
- Rollators and wheeled walkers
- Adjustable beds
- Manual wheelchairs
- Pressure care mattresses
Prescribed — Higher-cost equipment requiring OT or allied health assessment.
- Power wheelchairs and electric mobility scooters
- Advanced FES systems
- Ceiling hoists and transfer equipment
- Stairlifts and platform lifts
Home modifications include wheelchair ramps, doorway widening, bathroom modifications, and safety
flooring.
How to Access Your Funding — Step by Step
Step 1: Get assessed through My Aged Care
Call My Aged Care on 1800 200 422 or visit myagedcare.gov.au to register and arrange an aged
care assessment.
Step 2: Work with a provider and allied health professional
A clinical assessment — usually by an OT — identifies what equipment or modifications you need.
A quote is reviewed against your approved funding.
Step 3: Access your equipment
Once approved and a provider is confirmed, your provider notifies Services Australia and your
equipment is ordered or modification arranged.
Step 4: If funding isn’t enough
Your provider can request a Support Plan Review and submit evidence to access a higher tier.
